    Stopping plaquenil for surgery

In order to minimize the risks of infection and other potential complications with podiatric surgery in this patient population, this author reviews the literature and offers insights on whether medications for RA should be withdrawn, continued or modified during the perioperative period. Increasingly, the treatment of rheumatoid arthritis includes the utilization of immunosuppressive therapeutic agents.

Taking Plaquenil long-term or at high doses may cause irreversible damage to the retina of your eye. Stop taking Plaquenil and call your doctor at once if you have trouble focusing, if you see light streaks or flashes in your vision, or if you notice any swelling or color changes in your eyes. Three hundred and eighty-eight patients undergoing elective orthopedic surgeries were randomized to either continue or to stop MTX therapy for 2 weeks before surgery and resume 2 weeks postoperatively. Recently there have been a few cases in which patients of mine have been referred for orthopaedic procedures in the context of active Rheumatoid arthritis RA. The orthopaedic surgeon has expressed a real concern about the use of Methotrexate MTX in the peri-operative period, resulting in the patient being very anxious to stop the medication.

Louie and Ward demonstrated that in the state of California, rates of joint surgery for the treatment of RA declined in the 1990s and have continually diminished since that time.1 The authors concluded that the favorable long-term outcomes for the treatment of rheumatoid arthritis with newer agents have resulted in a decreased need for bone and joint surgery in patients afflicted with this disease. The effectiveness of disease modifying agents in reversing the progression of rheumatoid arthritis (RA) is illustrated by the reduced frequency with which patients with RA undergo surgery for bone and joint deformities.

  5. The issue raised is whether MTX must be discontinued temporarily in the perioperative period. It is my impression that most rheumatologists discontinue MTX during the perioperative period -- generally the week of surgery and for 1-2 weeks post surgery.

Plaquenil is a 6-weeker as far as the benefits are concerned, but it would be more dangerous to stop if you spend a lot of time outside and if this were the summer. If you're gonna experiment, I went off it cold turkey as it didn't hurt to do it that way, but you better get back on before the summer or you could experience a flare-up. Plaquenil is prescribed as a disease-modifier, in other words, to decrease pain, decrease swelling, and prevent joint damage and disability. It is not known how Plaquenil works but researchers believe that Plaquenil interferes with communication between cells within the immune system. Ultimately, it is thought to block proinflammatory. My rheumy's assistant left me a message to tell me to stop 2 weeks before my procedure, but didn't explain why. one more question for those of you who had surgery aside from going off or staying on plaquenil, did the surgery cause any noticeable flare up of your symptoms? i will certainly update you once i am back home and online again.
