    This is a copay assistance program for patients that have health insurance. The patient's insurance must cover the qualifying medication that they are seeking assistance for. Patient with Medicare Part D will be considered on a case by case basis. Patients must be at or below 400-500% of the federal poverty level. Speak to your prescribing physician and ask if the prescribed medication has a co-pay card assistance program or you can visit to find co-pay card assistance programs. Fill out the co-pay card enrollment form provided by your prescriber and send the form to the appropriate drug manufacturer. The patient must be getting treatment for rheumatoid arthritis. The patient must have Medicare health insurance that covers his or her qualifying medication or product. The patient’s medication or product must be listed on PAN’s list of covered medications. The patient’s income must fall at or below 400% of the Federal Poverty Level.
