Introduction Hydroxychloroquine is a Aminoquinoline derivative which is widely used for treatment of malaria, rheumatoid arthritis and lupus besides many other autoimmune diseases. Though neutropenia is listed as a potential side effect, there is no reported case in the literature. We present a very rare presentation of hydroxychloroquine toxicity. Neutropenia is a decrease in circulating neutrophils in the nonmarginal pool, which constitutes 4-5% of total body neutrophil stores. Most of the neutrophils are contained in the bone marrow, either as mitotically active one third or postmitotic mature cells two thirds. Hydroxychloroquine is classified as an anti-malarial drug. It is similar to chloroquine Aralen and is useful in treating several forms of malaria as well as lupus erythematosus and rheumatoid arthritis. Its mechanism of action is unknown. Malarial parasites invade human red blood cells.

Plaquenil is also an antirheumatic medicine and is used to treat symptoms of rheumatoid arthritis and discoid or systemic lupus erythematosus. This medicine is not effective against all strains of malaria.

Rheumatoid arthritis is a chronic disorder for which there is no known cure. Fortunately in the last few years, a shift in strategy toward the earlier institution of disease modifying drugs and the availability of new classes of medications have greatly improved the outcomes that can be expected by most patients. Neutropenia, also referred to as agranulocytosis, granulocytopenia, and granulopenia, is a decrease in circulating neutrophils, also known as granulocytes a type of white blood cell, in the peripheral blood. Neutropenia can be caused by a failure of the bone marrow to make enough neutrophils, or when white blood cells are destroyed faster than they can be made.
